1. Introduction to Person-Centered Care
In this chapter, we will introduce the 8 Principles of Person-Centered Care, discuss the agenda and learning objectives for this course, and present a scenario of an example resident you may encounter in your facility, to discuss how to incorporate person-centered care into your daily practice.
2. Learning Person-Centered Care Through Examples
With person-centered care, staff must work as a team to ensure that residents' needs are addressed with individualized approaches. The facility environment should be tailored to match the needs of the individual, rather than the facility guiding resident care. In this chapter, we will present three different example patients and identify how the 8 Principles of Person-Centered Care were used to improve their care experience.
3. Incorporating Person-Centered Care Into Your Work
Person-centered care has emerged as a priority for “good” care. However, measures for implementation are currently very limited in long-term care facilities. Instead, staff can use resident and family input to create a person-focused environment that may help to improve quality of life for residents. In this chapter, we’ll show a demonstration of a staff meeting to discuss improving a resident's care. We will also present some strategies for how to implement person-centered care in your facility.
