As evidence has definitively established a role for cognitive and emotional factors in the modulation of pain, the effective treatment of pain requires evidence-based methods to address these factors. This course introduces you to mindfulness-based treatment strategies for the active self-regulation of cognitive and emotional factors impacting pain. Mindfulness training has been shown to reduce catastrophic thinking, anxiety, and depression and improve positive affect in patients experiencing persistent pain. Strategies to support patients to broaden their attention beyond the sensation of pain and address worry, catastrophic thinking, and threat are introduced. The mindful investigation of pain is highlighted. Guidelines and handouts to introduce positive activities and mindful savoring of pleasant experiences are provided. Self-compassion and kindness meditation training are introduced, and a brief kindness meditation is demonstrated. Combining lecture, demonstration, and an experiential exercise, this course is applicable to healthcare professionals, including physical therapists, occupational therapists, speech-language pathologists, athletic trainers, and registered nurses, serving patients in both in- and outpatient settings who would benefit from sensible, effective pain self-management strategies.

Prerequisite: Previous knowledge of and experience with the principles and practices of mindfulness and their clinical applications to patient care are required to benefit from this course. It is highly recommended to watch the courses below in this order:

1. Mindfulness-Based Cognitive and Emotional Modulation Strategies

This chapter highlights the complexity of pain, reviews the Two Arrow metaphor, and introduces strategies to support patients to broaden their attention beyond the pain sensation and address worry, catastrophic thinking, and threat. The mindful investigation of pain is discussed.

2. Mindful of Pleasant Experiences

This chapter discusses the role of positive activities and mindful savoring of pleasant events in the treatment of pain and opioid misuse risk. The relationship between positive affect and endogenous opioid function is covered, and specific guidelines for engaging patients in positive activities and mindful savoring are presented.

3. Self-Compassion, Kindness, and Pain

This chapter introduces mindful self-compassion and kindness meditation training in pain treatment. Preliminary research is highlighted, an experiential exercise in kindness meditation is guided, and books for further exploration of this topic are offered.

4. A Patient’s Story, the Therapeutic Alliance, and Pain

In this chapter, a patient discusses her experience of training in mindfulness for a persistent pain condition. Mindfulness training to support the therapeutic alliance is introduced, and research on the power of the therapeutic alliance to modulate pain intensity and pressure pain threshold is discussed.
