We have the potential to close health disparity gaps, improve health and wellbeing, and reduce readmissions.

Health equity requires all individuals to have a fair and just opportunity to live a healthy life, and reaching this goal will require healthcare systems to take action and implement models that identify and address social determinants of health. By addressing patients’ social needs and changing the systems that facilitate inequity, we have the potential to close health disparity gaps, improve health and wellbeing, reduce readmissions, and stabilize healthcare costs.

In her new white paper, Breanna Lathrop, DNP, MPH, FNP-BC, COO and nurse practitioner at the Good Samaritan Health Center, offers a practical framework and resources for healthcare systems to better understand and address social determinants of health, improve the lives of their patients, and promote health equity.
