Social determinants of health significantly impact health outcomes and disparities but can be challenging to conceptualize and address in clinical settings. Strategies to identify and address social determinants have the potential shape our approach to healthcare in a way that centers patients' lived experiences and improves health outcomes. This webinar is designed for healthcare leaders and administrators who want to facilitate new strategies to promote health equity. Participants will gain a comprehensive understanding of social determinants of health along with tools and frameworks for identification of social determinants, intervention, and outcome management.

1. Why Address Social Determinants of Health?

This chapter defines social determinants of health within the context of health equity and discusses how social determinants of health impact health outcomes and disparities. It will also address why efforts to identify and address social determinants of health are relevant to home health in terms of clinical support, patient outcomes, and return on investment.

2. A Framework for Social Determinants of Health Work

Participants will receive a framework for addressing social determinants of health in the home health setting, including screening patients for social determinants of health and identifying common needs and disparities. This chapter will also provide guidance on program development and collaborative care networks as strategies to mitigate the impact of social determinants of health. This chapter includes an interview with home health expert and doctorly prepared registered nurse, Joanna Horst, on perspective taking and creating a patient-centered approach.

3. Leadership and Change Management

Incorporating a focus on social determinants of health requires change at both a system and clinician level which can be challenging and intimidating. This chapter provides guidance for organizational leaders on change management including facilitation, evaluation, and commitment to ongoing education around health equity.
