Pain Assessment and Management: Part II helps prepare the home care or hospice nurse to help manage pain medications. Common terminology related to pain management will be reviewed, including tolerance, dependence, and addiction. Managing drug-seeking behaviors will be covered as well as prescription drug monitoring programs and drug destruction. This course will also review non-opioid analgesics, adjuvant therapies, and opioids. Side effects and significant side effects will be discussed as well as titration and rotation of opioids for the management of pain and side effects. In addition to the review of medications, we will also discuss non-pharmacologic interventions for the management of pain in the home environment.

1. Basic Terminology

This chapter will review common terminology used in pain management. A strong clinical understanding of the difference between dependence, tolerance, and addiction is important in appropriate pain management. Additionally, some of the laws and principles that guide nurses in pain management will be reviewed.

2. Non-Opioids and Adjuvants

This chapter reviews the analgesic ladder and includes non-opioid analgesics and adjuvant therapies. The most common adjuvant medications will be reviewed along with a discussion of some adjuvant side effects, good and bad.

3. Opioids

This chapter reviews the mechanism of action for opioids. Weak and strong opioids will be reviewed. Titration is a common stumbling block for nurses, and a simplified method for application will be presented.

4. Opioid Management

Opioid management includes monitoring for effectiveness and for adverse effects. This chapter addresses common side effects as well as how to calculate the rotation of opioids.

5. Non-Pharmacologic Interventions

This chapter discusses interdisciplinary and nursing specific non-pharmalogic interventions. We will also discuss the identification and management of drug diversion. Common barriers to effective pain management will be reviewed.
