Demetra Antimisiaris (Dr. Dee) is the director of the University of Louisville's Frazier Polypharmacy and Medication Management Program. This program is dedicated to education, research, and outreach regarding polypharmacy. In addition to teaching Pharmacology to medical and dental students, Dr. Dee provides a weekly polypharmacy clinic and clinical consult service with the U of L Family Medicine and Geriatrics department. Her research interests include multi-stakeholder decision-making regarding medication use, and health systems analysis regarding polypharmacy.

Polypharmacy is a silent syndrome that often presents as functional and cognitive impairment, a new diagnosis, or treatment failure. Most health care providers recognize that polypharmacy is increasingly prevalent and have the understanding that it may be impairing optimal outcomes, yet, many health care providers feel powerless to manage and advocate for improved outcomes in patients living with polypharmacy, due to lack of formal training regarding this subject. Transitions of Care (TOC) and Medication Reconciliation (Med Rec) go hand-in-hand in potential for quality improvement as well as potential for patient harm. Med Rec and quality measures (QM) targets go hand-in-hand as well. This course will examine the potential impact of comprehensive patient-centered Med Rec on quality measures. We will discuss the impact of approaching TOC Med Rec beyond just getting the medication list right and the potential impact on reaping incentives beyond TOC codes. This course is not an update on billing codes, but is an in-depth look at patient-centered medication use assessment. The target audience is nurses (RNs), advance practice nurses (APRNs), nurse case managers, and nurse care coordinators.

Substance Abuse: Psychopharmacology and Treatment
Substance Abuse is becoming the predominant health epidemic challenge of the current era. In 2013, there were roughly 2,400 deaths due to substance abuse, and in 2016, there were over 50,000 deaths due to substance abuse. The most commonly fatal substances are analgesics, antidepressants, alcohol, central nervous system (CNS) stimulants, and cardiovascular agents. It is imperative that all healthcare providers understand the psycho-social dynamics linked to substance abuse, the pharmacology of substances of abuse, and available treatments. This course is aimed at improving healthcare professionals' and athletic trainers' skills regarding identification, characterization, and assessment of substance abuse.
