Case management is a vital part of our health care system that coordinates patient-centered care, providing cost-efficient care across the continuum. Health care systems ask for financial and clinical outcomes that are timely and use appropriate resources. For successful case management, a perspective of the whole person, diagnoses, expectations, treatments, resources and systems -- as well as assessments, plans, interventions and evaluations -- is required. This course will assist nurses and other health care professionals to implement best practices in case management.

1. History and Definitions of Case Management

This chapter presents a brief history of case management. Definitions from several organizations will be reviewed. The certification and the accreditation processes will be discussed. Standards of Practice will be addressed.

2. Goals of Case Management

This chapter will present general goals of case management. Specific goals may vary by setting and or client. The importance and implications of establishing specific goals, will be discussed.

3. Models and Roles of Case Management

This chapter will discuss models of case management set by employers or clients and the need for episodic or chronic management. Settings where these models may be used will be reviewed. Differences in role descriptions between facility based, insurance based or private case management will be presented.

4. Life Care Plans

Life care plans can be used to map out the needs of a person with chronic illness or disability over the remaining lifespan. The plans are also used in litigation to help determine costs and potential award amounts. This chapter will briefly discuss the living document that is created for long-term medical needs.
