Richard Briggs PT, MA has a clinical practice specializing in palliative care and hospice for the past 30 years. He has taught at the APTA National Conference and Combined Sections Meeting, the National Hospice and Palliative Care Organization (NHPCO) Clinical and Management Conferences, the California Hospice Foundation, and is Adjunct Faculty at California State University, Sacramento.

His articles have been published in Rehabilitation Oncology, Home Health Section Quarterly, NHPCO Insights and Complementary Therapies Handbook, Geriatric Physical Therapy 3rd Edition, and Topics in Geriatric Rehabilitation. He has served as chair of the NHPCO Allied Therapist Section and founded the APTA Hospice and Palliative Care SIG.

Questions frequently arise during care of seriously ill and declining patients, both from them and their family members. What should we say and how does a therapist talk about such sensitive issues in a context of both hope and reality? This course builds on the trust developed in the patient-therapist relationship to provide concrete examples of conversations around disease process, treatment effects, levels of exertion, energy conservation, and being open to possibilities of improvement and decline. We then move on to identify and explore the psychosocial and spiritual concerns that develop from the effects and losses of advanced disease states. The course concludes outlining sample documentation that justifies this supportive care in clinical therapy practice.

Therapists often work with seriously ill patients affected by trauma, aging, advanced disease or terminal illness. While it is essential that we bring the highest level of clinical care to each person, we must also bring our humanness; how we are can be as important as what we do. This course explores the psychological, social, and spiritual concerns faced by those in our care, as we accompany them along their path. Exercises include the experience and practice of mindfulness and presence for use as a clinical tool when issues of loss, suffering, and dying arise in patient care settings. The course concludes with a discussion of ways to support and cope with the losses both witnessed and experienced, so that rather than be depleted by this work, we might be fulfilled and become more whole.

Physical, occupational, and speech therapy are in their essence palliative, in that function and quality of life are maximized, rather than disease or disability cured. It is important to understand all aspects of the new hospice and palliative care niche to develop a successful and rewarding clinical practice. This course begins with the history and development of current methods, identification of philosophy, goals, organizational structure, and financial issues of the Medicare Hospice Benefit, the primary payer that mandates such care. The unique and essential roles of rehabilitative therapies are outlined, addressing practice patterns, team roles, equipment, and referral management. The course concludes with a review of cost neutral strategies for hospice therapy program development and optimal organizational success.

End of life issues are challenging to therapists when they arise in any setting along the palliative care spectrum: acute care, rehab, outpatient, home health, or hospice. Recognizing that a person is living with a life limiting condition significantly affects the therapeutic approach to care. This course will provide six practice models that can be applied to therapeutic intervention during the often variable clinical course, along with case examples. Clinical pearls that utilize our specialized knowledge and therapy skill set to maximize mobility and comfort for this patient population are discussed and demonstrated. The course concludes with a discussion of guidelines for the therapist providing end of life care to be successful.

How does a person with the decline inherent in aging, progressive disease, or a terminal illness continue to maintain mobility, safety, and independence in the face of the affronts to their physical ability and personal dignity? Falls, with the identification of risk and strategies for management during hospice, palliative, and end of life care, are the focus of this course. Patient and family engagement reveal areas of concern along with appropriate therapy assessment tools; clinical interventions achieve patient-centered goals. Particular attention is applied to unique transitions experienced during decline with practical education options. The course concludes with a discussion of the therapist role in supporting patient autonomy, family concern, and acceptable fall risks during the natural end of life path.
