Lisa Selby-Silverstein

PT, PhD, NCS

Dr. Selby-Silverstein is a Professor at Neumann University teaching in the Physical Therapy Program. She previously was on faculty at Thomas Jefferson University, where she also was Director of the Human Performance Laboratory. She has been a PT in rehabilitation settings, gait laboratories, developmental centers, early intervention, school-based, home care, and outpatient physical therapy settings. She received a Bachelor's Degree in PT from Ithaca College, a Master's Degree in Developmental Disabilities from NYU, and a PhD in Pediatric Physical Therapy from Hahnemann University, with research focused on foot and ankle biomechanics during gait and the effect of foot orthoses (conducted in the Gait Study Center of the PA College of Podiatric Medicine). She is a former National Institutes on Disability Rehab Research Switzer Fellow. Her research related to feet, orthoses, biomechanics, and gait has been funded, presented, and published. Most recently she has been active in APTA Evidence Based Practice Special interest group (Research Section), and the Foot and Ankle Special Interest group (Academy of Orthopedic Physical Therapy) serving as an author of the "Foot and Ankle Curricular Guidelines for Physical Therapist Professional Degree Programs." She will be re-certified as an ABPTS Board-Certified Neurological Clinical Specialist in 2019. She practices PT at Rainbow Rehab (Marlton & Mt Holly, NJ).

Overview of Orthoses and Alternatives to Improve Gait in Children

This course discusses types of orthoses, their componentry, and modifications that can be used to improve a child's gait. Alternatives to custom orthoses will also be discussed. Methods of adapting insoles and using Aquaplast(TM) and plaster casting to enable use of orthosis laboratories will be demonstrated. The therapist will be challenged to identify the problems to be addressed, the immediacy of need for remediation, the time period for orthosis use, and the environments where the orthosis is needed before deciding what type of orthosis and footwear to recommend. The need to do an orthotic checkout and modify the orthosis or footwear to optimize goal attainment will be highlighted. The fact that children will not wear the same orthosis all day in all settings and the concept of optimizing various footwear options for patients will be discussed.

This course presents intervention for and management of three children who used various orthoses: a child with Sever's Injury, a child with Spastic Diplegic Cerebral Palsy, and a child with Down Syndrome. Treatment options for a child with Sever's, including rigid taping, insole adaptations using Spenco, and Aquaplast alternatives (ranging from foot support to immobilization), will be presented. The implications of considering orthotic options but also assistive device options for children with cerebral palsy will be emphasized. Providing a plan of care that optimizes the principal of "least restrictive environment" and optimizing fitness will be emphasized by presenting PT management of a child with CP and a child with Down Syndrome.

This course reviews specific gait biomechanics to empower physical therapists to identify key problems and develop an effective plan of care, which may include an orthosis prescription, to improve a child's gait. It discusses how weight shifting, the multi-joint nature of many extrinsic foot muscles, and alignment of various body parts affects alignment and movement, proximally and distally. It also discusses how using an orthosis may affect a child's gait, mobility, sensation, strength, and range of motion. Understanding the factors described in this course are critical to developing an examination and intervention plan that addresses gait. The terms "center of mass" and "center of pressure" will be explained, and therapists will be challenged to envision the likely location of these points when watching a patient walk. Implications of location and motion of the center of mass and center of pressure have implications for understanding gait and foot and ankle motion and how they might be modified to improve gait.

This course presents examination/evaluation of 3 children whose management included orthoses: a child with Sever's Injury, a child with Spastic Diplegic Cerebral Palsy, and a child with Down Syndrome. An examination of lower body alignment will be demonstrated in a child with Sever's. Key biomechanical features of alignment and movement patterns common in Cerebral Palsy and Down Syndrome will be discussed. The importance of focusing on the child's goals related to lifting their limitations in functional activities and participation will be discussed; working backward to identify potentially modifiable factors will be used to help the therapist prioritize examination components. Another goal of intervention will be to minimize the development or progression of deformity that could ultimately limit function. The need to balance optimizing current function with enabling holistic developmental progression and fitness while preserving hip location and minimizing long-term stresses on knees, feet, and backs will be discussed.
