Video Runtime: 97 Minutes, Learning Assessments: 38 Minutes

The Centers for Medicare & Medicaid Services (CMS) released their 2020 rules regarding reimbursement of rehabilitation: including executing and documenting the modified barium swallow (MBS). The purpose of this presentation is to demonstrate how an entire healthcare system implemented these standards. All radiology services were updated utilizing standardized products, equipment, and recording systems. All speech pathologists were educated in and personally observed when executing and integrating these standards in the radiology suite, performing documentation, and communicating results of the study. Practice patterns were implemented by mirroring the recommendations from the ASHA dysphagia portal. This presentation will translate ASHA's recommended MBS standards of dysphagia practice to maximize delivery of patient- and value-driven dysphagia services across a healthcare system.

Video Runtime: 78 Minutes, Learning Assessments: 44 Minutes

Speech pathologists and other members of the rehab team are faced with growing caseloads of patients with remarkable medical disorders that need to be assessed and treated efficiently. We are asked to see patients in the intensive care unit with significant pulmonary issues, who may be on various oxygen delivery systems, including noninvasive ventilation. Participants will learn pulmonary sequelae of pathologies and how to manage these medically fragile patients. Understanding how these manifestations may interfere with normal swallowing and the goal of our clinical swallowing evaluation and instrumental and/or treatment options is imperative. This course will increase your critical thinking skills for assessing and treating adults with respiratory disorders.

Video Runtime: 57 Minutes, Learning Assessments: 31 Minutes

Speech pathologists and acute care rehab clinicians are faced with growing caseloads of patients with cardiac disorders and the sequelae of disease progression. We are asked to see patients in the intensive care or cardiopulmonary unit with acute or chronic cardiac disease. In this course, participants will learn about cardiac anatomy/physiology, diseases, and how these issues can interfere with everyday living, including mobility and eating and drinking safely. Patients face decisions on how to treat their diseases, first empirically and then with possible surgical interventions. Cardiac catheter interventions and surgical interventions will be discussed. Current research regarding why patients may experience post-op swallowing disorders will be covered, as well as how to implement a pre- and post-op swallow screen in your institution. This course will increase your critical thinking skills for assessing and treating patients with adult cardiac disorders.

Screening is the first step in the continuum of dysphagia assessment. There are many different approaches to screening, and clinicians are often unclear about which one to use. In this course, we will discuss methods of screening, how to choose the best screening approach, and how to implement screening in various healthcare settings. Clinical swallow evaluations are often the next step after screening. In this course, we will discuss components of a clinical swallow evaluation. We will delineate what can and cannot be ascertained regarding swallow function during a clinical swallow evaluation. Finally, we will discuss how screening and clinical swallow evaluation can be incorporated in various healthcare settings, including intensive care, inpatient rehabilitation, and outpatient settings.

Video Runtime: 42 Minutes; Learning Assessment Time: 31 Minutes

There is a high need for integration and collaboration in the provision of speech pathology services for hyperacute stroke patients. Primary stroke centers (PSC) and comprehensive stroke centers (CSC) are dedicated stroke-focused programs with integrated ER to ICU trends and bundles for best practice. This requires a team approach for advanced imaging, 24/7 available personnel, ICU/neuroscience, ICU facilities, and capabilities of experienced personnel in treating patients with a variety of strokes. Speech pathologist involvement with hyperacute stroke patients is paramount to initiate evaluations and treatment at the time of insult post tPA and/or thrombectomy. Collaboration and communication with the rehabilitation team are key for patient-centered care. Speech pathologists are highly suited to educate patients and their families regarding communication, cognition, and swallowing status at the onset of injury.

Due to the high prevalence of strokes in the United States, there is a high need for integration and collaboration in the provision of speech pathology services for stroke patients. Integrated MD, RN, physical, occupational, speech, and social work/case manager services are best practice for a primary or comprehensive stroke center. Speech pathologist involvement with acute stroke patients is paramount to initiate evaluations and course of treatment at the time of admission to the hospital, hopefully on the acute neuro unit. Collaboration and communication with the rehabilitation team are key for patient-centered care. An integrated team approach helps to decide if a patient can tolerate an intense inpatient rehabilitation program, skilled nursing facility, or home with home health/outpatient therapy. Speech pathologists are highly suited to educate patients and their families regarding communication, cognition, and swallowing status post stroke.

Subscribe now, and access clinical education and patient education—anytime, anywhere—with video instruction from recognized industry experts.
Video Runtime: 73 Minutes, Learning Assessments: 41 Minutes

There is an extensive need for integration and collaboration for the provision of all rehab services in the ICU. Patients with acquired brain injuries, including but not limited to concussion, mild to severe TBI, stroke, brain tumors, and head bleeds benefit from integrated rehabilitation evaluations. Speech pathologists are crucial for assisting the ICU team to focus on patient-centered care, communication with the medical team, and educating the patient/family regarding the next level of care. Speech pathologists are needed at the initiation of the patient's insult to provide communication with the patient and their families throughout the continuum of care. Speech pathology evaluations should encompass communication, cognition, and swallowing, which is paramount in the initial stages of an acute brain injury; that and extensive family education provides a more successful transition for the patient's next level of care. Any speech pathologist that works with acquired brain injuries is an applicable audience for this course. This course will provide an overview of acquired brain injuries from mild to severe, evaluation and documentation practices for communication, cognition, and swallowing. Strategies for effective interdisciplinary communication and collaboration is reviewed.

A team approach is the best method to effectively integrate best practices with ICU trends and bundles. Speech Pathologists involvement in the ICU and their understanding of A-F bundles are paramount to initiate evaluations and treatment at the onset of illness to ensure patient-centered care.

This course will cover the prevalence and evidence of ICU trends and bundles, how patients benefit from communication and swallowing services, and how to deliver adult ICU evidence based patient-centered care. A summary will be provided on how Speech Pathologists assist patients with communication needs while intubated by utilizing nonverbal methods. Strategies for collaboration with RNs on managing post-extubation dysphagia and the need for implementing evidence-based swallow screens will be reviewed. This course will walk through the Speech Pathologists' role in educating patients and their families regarding post-intensive-care syndrome and how to pragmatically work through the patients' cognitive, emotional, and physical difficulties due to the harsh ICU environment. This course is applicable to any healthcare Speech Pathologist.
