Jennifer T. Dodson

OTD, OTR/L, CHT

Jennifer Dodson graduated from the Florida International University occupational therapy program in 1998. She has been practicing outpatient rehabilitation in Jacksonville, Florida, ever since. In 2002, she was honored to be chosen as a fellow for the Evelyn Mackin Hand Therapy Fellowship at the Philadelphia Hand Center. She became a certified hand therapist in 2004. Part of the mission of the fellowship was to have the fellow bring learned knowledge to an underserved area. To help fulfill this mission, she created her own continuing education company, 3Kings Upper Extremity Specialists, specializing in upper extremity rehabilitation. 3Kings Upper Extremity Specialists brings quality continuing education courses to Jacksonville with the support of local hand/plastic surgeons and CHTs from around the country. Additional goals of starting her own company were to unite the CHTs and MDs in the Jacksonville area, to promote evidence-based practice by holding journal clubs, and to mentor area therapists.

Jennifer is an active member of the American Society of Hand Therapy and has served on the education council. She has been invited to lecture at local and national conferences around the US and Canada on various upper extremity topics. She currently works at UF Health Jacksonville, a level 1 academic trauma hospital. This allows her to practice in the two areas in which she is passionate: trauma and sports medicine. She previously served as a clinical hand specialist, where she developed the hand therapy program at the hospital. Through her years of practice, she has had the ability to start up hand therapy programs in various settings.

Shoulder pain is a common diagnosis for which therapists get referrals that limit a person's ability to use their arm, especially in overhead activities. Therapists may also receive referrals for diagnoses distal to the shoulder but will observe pain in the shoulder when patients return to use of the entire upper extremity. Therapists must be knowledgeable in anatomy/biomechanics/arthrokinematics of the shoulder, be aware of the special tests to perform, and use patient-reported outcome measures to establish an effective treatment plan for those with shoulder impingement syndrome.

In this course, therapists will learn the different types of shoulder impingement and review biomechanical studies/arthrokinematics, special tests to perform, and assessment of soft tissue/joint imbalances when evaluating patients with shoulder pain.

Therapists often get referrals for shoulder impingement syndrome, a common diagnosis that limits an individual's ability to use their arm with meaningful occupations. They may also receive referrals for diagnoses distal to the shoulder but observe pain in the shoulder when the patient resumes use of the entire upper extremity, or they may be the first to identify shoulder dysfunction. Once a comprehensive evaluation is performed, therapists must choose evidence-informed interventions to address the impairments identified to return patients to meaningful occupations. There are many factors that can contribute to shoulder impingement syndrome, and treatment interventions must be individualized to meet each patient's unique needs. Brachial plexus injuries are devastating injuries that affect all aspects of a person's life. The complexity of these injuries requires a specialized team-based approach to address the multifaceted aspects of returning the patient to meaningful occupations. Advancements in nerve transfers restore hope and function in those who have sustained brachial plexus injuries. Just as timely surgical intervention is essential to optimal recovery, pre- and postoperative rehabilitation play a distinct and vital role in maximizing outcomes. Although there is a lack of high-level studies demonstrating the ideal postoperative management of these injuries, a few key articles are beneficial for therapists treating these injuries to become familiar with. These articles provide foundational knowledge and concepts that assist therapists in developing an individualized rehabilitation program. Synthesis of these articles will be presented along with a case study that will allow therapists to understand the importance of a comprehensive evaluation, the criteria to progress the rehabilitation program, and interventions to address the multifaceted impairments this population presents with. This course will review considerations a therapist must address during a comprehensive evaluation of traumatic/complex elbow injuries. Therapists must also be knowledgeable about the types of complex elbow fracture diagnoses they may see in documentation or on therapy prescriptions, to determine what structures are involved. To begin the course, a review of the wound healing process as it relates to all structures involved will be presented. Following this, pertinent biomechanical studies will be reviewed that will impact decisions made by therapists through all phases of rehabilitation. Lastly, special considerations on emerging areas of practice will be discussed that can assist the therapist in developing a more client-centered treatment plan. In the literature, various terms are used to describe complex traumatic injuries. Due to this variability, the injuries in this course will address those that are a result of trauma that involves damage to two or more of the following structures: soft tissue (skin/fascia), bone, muscle, tendon, ligament, blood vessels, and peripheral nerve injuries.

After a comprehensive evaluation of the traumatic/complex elbow is performed, therapists must implement therapeutic interventions that are appropriate based on the stage of wound healing the patient is in. Therapists must also use clinical reason-ing to modify treatments according to patient factors and clinical presentation. Evidence-based decisions, along with critical reasoning throughout all stages of rehabilitation, are essential to maximize return to meaningful occupation. This course will provide treatment ideas based on the stages of wound healing and the patient's presentation, with emphasis on client-centered care. Therapeutic exercises, ADL training, manual interventions, and orthosis use are some areas that will be ad-dressed. Lastly, as therapists play a vital role in whether or not patients adhere to their home programs and post-operative restrictions, suggestions to increase patient adherence will be introduced to maximize functional outcomes and patient satisfaction.
