This course builds upon the principles from the Home Health QAPI - Part 1 course. The focus of Part 2 is to provide guidance and strategies around the final QAPI standard, Performance Improvement Project (PIP). An interdisciplinary PIP team will be selected to brainstorm on potential evidence-based interventions to address the specific problem or improvement area identified from agency data. Testing, spreading, and sustainment of the PIP interventions is critical to the success of the project. In addition to learning key principles and strategies in this course, there will be a demonstration of a PIP team in action.

1. Performance Improvement Project (PIP) Overview

Home Health Quality Assessment & Performance Improvement’s (QAPI) five standards include the development of a Performance Improvement Project (PIP). Review the purpose, goals, guidance, and types of PIPs. Discover that PIPs are the workhorse of quality improvement.

2. PIP Preparation

PIP development required a systematic approach. Leadership needs to complete a variety of activities before planning interventions. Data aggregation and analysis identifies potential root causes of the problem area. The PIP team selection needs to be thoughtful and interdisciplinary.

3. PIP Development

Explore the basics for developing PIP using a team approach and quality improvement techniques. Learn how to leverage evidence-based practices including tools and resources. Develop rapid-cycle testing of changes before spreading within organization to validate improvement.

4. PIP Demonstration

Each phase of a PIP will begin flowing into an action plan. In this chapter we will use a PIP example showing the principles and evidence-based practices are being integrated. Learn how a PIP team leader can jumpstart a team into action.

5. Summary & Resources

Learn more about situations where an agency may need to develop more than one PIP in a year. The PIP continues after implementation with monitoring, evaluation, and sustainment. Review and apply key points and resources for PIP development and documentation.
