This course will examine the publicly reported Home Health Star Ratings system. There are two different sets of reports that include process, outcome, and patient satisfaction measures. The learner will explore what the ratings mean, data sources, effects on referrals, and how to develop a Performance Improvement Plan (PIP) to aim for 5 Star Ratings.

1. Home Health Star Ratings Measures

The Centers for Medicare & Medicaid Services (CMS) publically reports home health data including Star Ratings. Learn how the ratings are being used by patients and referral sources. Explore which measures and the data sources are used for the Quality of Patient Care and Patient Survey Star Ratings. Learn how to use CMS tables to understand data source amd inclusions and exclusions criteria for STAR measures.

2. Data Drives the Path

Quality Assurance & Performance Improvement (QAPI) process is foundational to improving Star Ratings. The QAPI process begins with collecting and trending data. This chapter will explore available data resources and develop a plan for collection. A discussion of several considerations when prioritizing STAR measure(s) for quality improvement.

3. Interdisciplinary Team & the Evidence-Based Interventions

Learn about attributes to create an interdisciplinary team for a specific Performance Improvement Project (PIP). Clinical champions are necessary to test and refine evidence-based interventions or best practices that will improve Star Ratings. Discover where to locate intervention, tools, and resources that are appropriate for community-based patients.

4. Test & Spread

It is essential to use rapid-cycle testing (e.g., Plan-Do-Study-Act) with PIP. Explore how to run multiple PDSA cycles to fine tune the intervention changes. Watch a short video on PDSA. The next step is to spread the intervention, but one size (PIP) does not fit for all teams or offices. Learn how to modify the PIP to spread and achieve success across the organization.

5. Sustainment

The PIP continues after implementation with monitoring, evaluation, and sustainment. It is critical to measure progress and continually use strategies to keep clinicians engaged with new practices and progress toward goal. Leadership’s recognition and reward will support the sustainment of the PIP.

6. Summary & Resources

Trend and monitor data for selecting the next home health Star measures for PIP development. Review key points and resources to continue towards achieving 5 Stars status.
