This course will explain the phenomenon of pain that persists after the expected amount of time it should last, otherwise known as chronic pain. This course will address the impact of chronic pain on the patient and the clinical team providers. Lastly, the course will discuss the dilemmas of long-term opioid use and the potential for tolerance, dependence, and abuse. This course is most applicable to nurses, occupational therapists, and physical therapists in acute care, inpatient rehabilitation, long-term care, and home health settings.

This chapter will discuss the maladaptive pain experience, including chronic neuropathic pain, and the phenomenon of chronic noncancer pain. The chapter will also explore the impact of social, emotional, and psychological factors of chronic pain on the patient and the family.

2. Long-Term Opioid Use for Persistent Noncancer Pain

This chapter will explain the most common types of medical conditions that involve chronic noncancer persistent pain. In addition, the chapter will review the goals of treatment for long-term noncancer pain conditions and the current prescribing guidelines, including absolute and relative contraindications for providers and clinicians to consider.

3. Ethical Issues: Tolerance, Misuse, and Addiction

This chapter will explore the long-term dilemmas of opioid use and define tolerance, misuse, and addiction. This chapter will delve into the potential pitfalls of chronic opioid prescribing and caring for patients on long-term opioid therapy, and it will discuss the current opioid crisis.
