Persons with multiple sclerosis (MS) are being referred to therapy in increasingly greater numbers. However, the training that most clinicians receive in school is frequently inadequate to meet the needs of these patients and often results in poor outcomes and reluctance on the part of physicians to refer these patients to therapy in the future. The purpose of this course is to present the most current evidence on the topic of rehabilitation interventions for persons with MS, as well as to provide a clear understanding of the neuropathology and how it contributes to a unique presentation, followed by a description of common impairments and functional limitations. As MS is a disease that results in progressive neurologic degeneration, information regarding management of the disease over the spectrums of time and disability will also be emphasized. Examination and intervention strategies for both common and uncommon presentations will be discussed. Case studies will be used to emphasize the spectrum of disability.

Herb Karpatkin, PT, DSc, graduated from the physical therapy program at Boston University's Sargent College in 1989 and received his doctoral degree in neurologic physical therapy from Rocky Mountain University of Health Professions. 1. Pathophysiology and Medical Management

In this chapter, learners will be briefly exposed to the unique pathophysiology of multiple sclerosis and how it results in unique and varied presentations. This will be followed by a brief discussion of medical management and how this management can impact therapy practice.

2. Evaluation and Assessment

In this section, classification and evaluation of multiple sclerosis will be presented. Global evaluations will first be presented, followed by evaluation procedures to examine impairments and functional limitations specific to rehabilitation outcomes. Upper extremity, trunk, and lower extremity impairments, as well as ADL and functional limitations, will be covered. Assessment and prognostication of the evaluative findings will be addressed.

3. Intervention Strategies and Case Example

In this section, treatment interventions will be discussed. Emphasis will be paid to treating the specific findings of note in the evaluation using the available peer-reviewed evidence as well as expert opinion. Case examples will be used to illustrate specific areas of clinical emphasis.
