More than 77 million baby boomers have reached their 70s, according to a 2017 nationwide survey. An aging population is often associated with complex medication regimens and negative effects that are too frequently ignored. Polypharmacy is a growing concern in our health care system, both in elderly and other special populations. This course is the second part of a two-part series and should be taken after Part One has been completed. This course discusses methods and solutions to overcome challenges faced in polypharmacy, including the medication reconciliation and deprescribing processes. There is an in-depth discussion on how to use evidence-based tools such as the Beers Criteria to reduce polypharmacy and assess for potentially inappropriate prescribing. A relatable case review is included. This course is applicable to all health care professionals who interact with patients in home health and/or hospice settings.

1. Reducing Polypharmacy: Part One

In this chapter, several strategies to reduce the negative effects of polypharmacy are discussed. A case is introduced and is revisited throughout the duration of the course. The basic steps to a medication reconciliation process are provided. How to deprescribe successfully is also presented.

2. Reducing Polypharmacy: Part Two

This chapter further discusses strategies to reduce the negative effects of polypharmacy using evidence-based tools, including the Beers Criteria, STOPP/START, and MAI.

3. Case Analysis

In this chapter, the case is revisited with a thorough analysis of what was discussed in the course overall. A course summary is provided.
