Medication-related problems are common among patients, especially those who have multiple disease states and complex medication histories. The management of patient medications can be a challenge for all health care practitioners in both the hospital and home care settings. This course will discuss the need for conducting a thorough and effective medication reconciliation. The steps to perform a medication reconciliation will be discussed in detail, including how to conduct a structured patient interview. Tips on how to make the medication reconciliation process more effective will be provided, and relatable case reviews will be demonstrated. This course is applicable to nurses, therapists, social workers, and any other health care professionals interacting with patients in home health and/or hospice settings.

1. Case Scenario and Course Overview

In this chapter, an applicable case scenario will be introduced that will be revisited throughout the course. An overview of the course and significant statistics are provided. The learning objectives are also presented.

2. Key Terms and the Rationale for Medication Management

In this chapter, several key terms are explained. There is a brief discussion on medication errors and adverse drug events (ADEs), how they are related to one another, and tips on how to prevent them during the medication reconciliation process. The purpose of the medication reconciliation process is addressed. The learner is able to apply what was discussed in this chapter to the case scenario introduced in Chapter One.

3. How to Perform an Effective Medication Reconciliation

The basic steps of a medication reconciliation are provided, with an in-depth discussion on how to make the process of medication reconciliation effective and how to perform a thorough patient interview. The course is concluded with a final visit back to the case scenario.
