This chapter will explore the scope of chronic disease using the International Classification of Functioning, Disability and Health (ICF) and social determinants of health to identify and manage the contextual factors increasing the burden of illness in older adults.
2. Integration of Coaching Principles
This chapter will explore key applications of Self-Determination Theory, the Transtheoretical Model of Behavior Change, and motivational interviewing to support quality of life in older adults with chronic illness.
3. Capacity Coaching for Persons With Multiple/Chronic Conditions
This chapter will utilize a case study for the exploration and application of capacity coaching, an emerging strategy for reducing the challenges of chronic illness and treatment.
