Chronic, lifestyle-related disease is an epidemic in the US. Adults 65 and over are disproportionally affected, with 85% living with multiple conditions. Health coaching has significant evidence supporting its value in chronic disease management. This course will enable physical therapists and occupational therapists to apply evidence-based coaching strategies, especially the emerging strategy of capacity coaching, to improve the quality of life in older persons living with multiple conditions.
