Advanced/late-stage dementia is a period characterized by immobility, loss of purposeful movement, dysphagia, and total dependence upon others for all their care needs. This course will discuss key information, techniques, and best practices for home health care professionals in meeting the needs of patients with advanced/late-stage dementia. Palliative care, behavioral challenges, and best practices for managing functional decline will be discussed. Additionally, planning strategies for the future will be presented.

1. Advanced/Late-Stage Dementia: The Palliative Care Approach

Advanced or late-stage dementia marks the time of significant decline in the individual with dementia. Palliative care is essential during this time as the patient’s physical and mental health needs increase. In this chapter, the impact of advanced/late-stage dementia on functional ability is discussed, including ways that the medical care provider team can provide palliative care for the patient.

2. Behavioral Challenges of Advanced/Late-Stage Dementia

This chapter provides an overview of the behavioral challenges associated with advanced dementia, and discusses nursing interventions to support the patient during the last one to three years of life. Strategies to enhance patient quality of life will also be discussed.

3. Managing Functional Decline

The last years of life are the most debilitating for the patient with dementia and their family. In this chapter, techniques for managing functional decline will be reviewed, including how to address activities of daily living (ADL). Education and support approaches will be presented and discussed.

4. Planning for the Future

This chapter will review long-term planning, including advance directives, respite care, evaluating home safety, and expected outcomes. Lifestyle and pleasant events will be discussed, as well as ways to ground activities in sensory stimulation.
