1. Assessment and Problem Identification
This chapter will review the process of information gathering and analyzing the data. Consideration of community and financial resources will be discussed.
2. Plan of Care
This chapter will discuss determining the plan of care using the problems that have been identified. An interprofessional team approach that will meet the needs of complex patients will be discussed.
3. Implementation and Coordination
This chapter will work through the implementation of the plan, including coordinating care and interprofessional communication. Discharge planning and use of community resources will be discussed to promote positive outcomes.
4. Evaluation
Through case study examples, this chapter will help to evaluate the progress and success of the case management plan. Goals are not always accomplished, so the need for revising plans will be reviewed. This chapter will also present an interprofessional panel of experts who will discuss assessments, planning, and coordination for a population with chronic care needs. Plans for equipment, ongoing therapy, and education for caregivers will be discussed.
