Obesity now affects an estimated 36.7% of adult Americans. In addition to its own negative impact on health, obesity-related conditions further add to morbidity and mortality, rendering obesity a significant public health issue. As new, improved surgical techniques have been introduced and their outcomes reported, guidelines and recommendations from various health organizations have supported metabolic and bariatric surgery as an effective, cost-effective treatment option for those with obesity who meet specified requirements. This course will provide the participant with information regarding current guidelines/recommendations for metabolic/bariatric surgery, surgical procedures currently approved, and pre- and postoperative care of the patient undergoing this surgery. A case study is included to allow participants to synthesize and apply knowledge attained. This course is appropriate for nurses and other health care providers in any setting who care for individuals who have undergone or may undergo metabolic/weight loss surgery.

1. Introduction to Metabolic and Bariatric Surgery

Metabolic/bariatric surgical techniques have evolved greatly in recent years. This chapter will provide an introduction to current recommendations and guidelines for these surgeries. Data regarding approved surgical techniques, including utilization, weight loss outcomes, effects on comorbidities, complications, and related issues will be presented.

2. Currently Approved Metabolic and Bariatric Surgical Procedures

Several metabolic/bariatric surgical procedures are approved for the treatment of obesity. This chapter will provide a review of these procedures and their effects, and compare the procedures in terms of success rate/weight loss potential, complications, and other considerations. Immediate, postoperative, and long-term patient care/concerns will be discussed.

3. Pre- and Post Operative Care of the Patient Undergoing Metabolic and Bariatric Surgery

Preoperative workup, patient education, and other preoperative preparations aid in the identification of individuals appropriate for these types of surgeries and help to ensure successful surgical outcomes. In the same manner, postoperative monitoring, patient education, support, follow-up, and early identification and intervention of any complication(s) are essential for attainment of the desired weight loss outcome. This chapter will present common preoperative and postoperative regimens; identify potential complications, including cause(s), symptoms, and appropriate interventions; and provide the learner with the knowledge and skill to plan for and provide safe, sensitive, and effective care. Opportunity to analyze a case study of a person undergoing metabolic/bariatric surgery will be provided.
