Measuring and Improving Outcomes in Post-Acute Care

This course examines the current state of rehabilitation outcomes measurement in post-acute care (PAC) settings: inpatient rehabilitation, skilled nursing facility, long term acute care and home health care. Patients cared for in these settings often transition between multiple sites of care when their health and functional status changes, and therefore are particularly vulnerable and costly to the health care system. Currently, performance measurements across PAC settings are fragmented due to the heterogeneity of patient populations, as well as the various reporting mechanisms across settings. This course provides participants with a context for understanding the complex issues that contribute to the questions raised by external agencies and policymakers who seek to understand the value of rehabilitation in post-acute care. Instructor Ellen Strunk describes the efforts currently underway to gather more information and develop meaningful outcome measures to support the value of rehabilitation services in disease management as well as future models of payment. Universal concepts of the current state of rehabilitation outcomes and utilization measurement at every level of rehabilitation delivery are discussed. Outcomes measurement, data collection and analysis of information contribute to the value equation necessary in today’s healthcare environment. This course discusses some elements of the federal Medicare benefits and payment models which are under constant change and may no longer apply. Understanding Counseling and the Process of Change

Speech-language pathologists who work with individuals who stutter do more than simply teach speech techniques. Because of the complexity of the stuttering disorder, therapists often find themselves addressing both attitudinal and emotional aspects of the speaker’s experience of stuttering. Unfortunately, many clinicians report that they are less comfortable working with these components of the stuttering disorder. This course (part one of three in this course series) will discuss some of the key aspects of the counseling process that clinicians can apply to their work with individuals who stutter. Specifically, the course will provide clinicians with a high-level overview of a model of change that can help them guide their clients through the process of therapy so they can achieve their best possible outcomes from intervention. Phonomotor Treatment for Individuals with Aphasia: Evidence Based Practice

The purpose of this course, with Dr. Diane Kendall and Megan Oelke, is to provide information regarding the development of and evidence for the Phonomotor Treatment Program for rehabilitation of language deficits in aphasia. The course will include background on and a definition of of phonology in aphasia, a review of the model upon which the treatment was based, Phase I and II evidence. The course will also include an in-depth tutorial on how to deliver phonomotor treatment, including a description of materials, cueing hierarchy, stimuli employed, feedback techniques and how to progress through the stages of treatment. The course concludes with relevant tips for clinicians, and practice examples. Learn more
